Personal Information Collection Statement (“PICS”)

 

We shall keep your Personal Data confidential at all times. Our policies and practices with respect to the collection, use, retention, disclosure, transfer, security and access of Personal Data will be in accordance with the laws of Hong Kong and are as set out in this PICS.

 

You agree that all the Personal Data collected by us or provided by you as above may be used and retained by us for all purposes in relation to:

 

(i) To access your medical records and transmit medical reports for the purpose of providing necessary medical care and services; and

(ii) To share your medical records with a third-party referral doctor as required for referral purposes.

 

You agree that we could disclose and transfer your Personal Data (including your name and your portrait) to our other subsidiaries, affiliates and shareholders for use in maintaining accurate medical records and ensuring comprehensive assessment in the future.

 

We may disclose and transfer (whether in Hong Kong or abroad) to our agents or contractors under a duty of confidentiality to us who provide administrative, data processing, research and marketing, distribution, telecommunications, professional or other similar services to us and to any of our actual or proposed assignees or transferees of our rights with respect to you in connection with a merger, sale or transfer (whether of assets or shares), to use, hold, process or retain such Personal Data for the purposes mentioned above on our behalf.

 

You also agree that we could disclose, transfer or grant access (whether in Hong Kong, Macau or mainland China) to share your Personal Data (including your name, date of birth, medical records / reports and insurance coverage) to our subsidiaries, affiliates, associates, holding company, business partners, as well as employers, brokerage firms and insurance companies that have business dealings with UMP for purposes as follows:-

 

(a) For identification and verification purposes in connection with the medical and/or health care and services that may be supplied to you;

(b) For maintaining accurate medical records;

(c) For rendering medical and/or health care and services to you; and

(d) For any investigations relating to your insurance policy, your employee health benefits plan, and your dependent coverage under any employee health benefits plan.

 

With your consent, we could use your personal data collected by us (including your name and contact details) for direct marketing in relation to the medical and/or healthcare service (the “Services”) of UMP Healthcare Group (“UMP”).  UMP may also disclose, transfer or grant access (whether in Hong Kong or abroad) to share your

personal information (including your name and contact details) to any of its platforms,

 

subsidiaries, affiliates, associates, holding company and business partners for their use in direct marketing in relation to the Services after obtaining your consent.

 

If you agree to receive marketing communications but do not wish to receive them in the future, you may opt out of receiving them at any time, free of charge, by the following applicable means:

(a) unsubscribing by following the “Reply to” instructions contained in the marketing text message;

(b) following the unsubscribe instructions or hyperlink in the message;

(c) following the unsubscribe instructions in mobile applications;

(d) notifying us that you no longer wish to receive marketing communications when receiving our marketing calls; or

(e) contacting us in writing by post or by email at the address or email address stated herein below to tell us that you no longer wish to receive marketing communication.

 

You have the right to:

(a) access, correct and delete of any Personal Data held by us;

(b) ascertain our policies and practices (from time to time) in relation to Personal Data and the types of Personal Data held by us.
